16 oz black plastic stadium cup

I need a quote for 16oz, plain black, smooth wall stadium cups (1 color line art, on 2 sides) packaged in cases of 500 ea, in sleeves of 50 ea. Quote: <50 cases; 100 cases, 200 cases, 500 cases, and >500 cases.

Quote separately the cost of molding these with 3 to 4 1/2" holes placed around the top edge of the cup with the bottom edge of the hole 1" from the top of the cup.

Quote separately the cost of printing and affixing a water proof bar code label to the side of each cup, adjacent to the bottom edge of the cup.

Target weight of cup is 20-22 grams.

Relevant suggestions that can assist in lowering costs or improving quality should be included.
