Piracetam is nootropic agent, that is a psychotropic drug which improves the functioning of the brain involved in cognitive processes e.g. memory, thorough learning in normal and in subnormal conditions. The beneficial effects are due to improved microcirculation, promotes the metabolism and modulates neurotransmission. Protect cerebral cortex against hypoxia.

Composition

active substance: piracetam;

1 ml of solution contains **0 mg of piracetam;

excipients: sodium acetate trihydrate, water for injections

Indications

• Symptomatic treatment for psyhoorganic syndrome associated with disturbances of memory and attention, dizziness;
• Mono-or combined therapy of cortical myoclonia;
• Dizziness and related balance disorders, except for dizziness of vasomotor and psychic origin;
• In patients with chronic alcoholism for the treatment of psyhoorganic and withdrawal syndromes;
• Treatment of speech disorders (dyslexia) in children over 8 years of age in combination with other appropriate therapies.

Contraindication

• Hypersensitivity to piracetam or pyrrolidone derivatives, as well as other components of the drug;
• Hemorrhagic stroke;
• Acute renal failure (creatinine clearance less than *0 ml/min);
• Huntington's chorea;
• Children under 8 years of age;
• Pregnancy and lactation.

Dosage and administration

An injectable formulation of the drug should be used for acute cases or when the use of other pharmaceutical forms of piracetam is not feasible. The medication is administered intramuscularly or intravenously (injected slowly over a period of several minutes), starting with 2 - 4 g and quickly increasing the dose up to 4 - 6 g daily. In adults the daily dose is *****0 mg/kg of the bodyweight (in **3 divided doses). After clinical improvement the patient should be transferred to oral therapy with piracetam.

Symptomatic treatment for psyhoorganic syndrome: the daily dose is 2.4 4.8 g in **3 divided doses.

For the treatment of cortical myoclonia the recommended starting dose is *4 g daily (in 2 3 divided doses) during the first 3 days. If the therapeutic effect is lacking or insufficient, the drug should be used in the same dose for another 3 - 4 days. If the desired therapeutic effect is lacking after 7 days of the treatment with piracetam, the drug should be discontinued. In case of a positive therapeutic effect, the treatment is continued. The drug should not be stopped abruptly; the dose should be reduced gradually by 1 - 2 g every **3 days, in the case of Lance-Adams syndrome - every 3 - 4 days. Retreatments with piracetam for acute attacks of the disease should be performed every 6 months, adjusting the dose of the drug accordingly.

For the treatment of vertigo and related balance disturbances, the daily dose is 2.4 - 4.8 g in 2 - 3 divided doses.

For the treatment of alcoholism: in acute phase - *2 g daily, the maintenance dose - 2.4 g daily.

Combined therapy of speech disorders in children: a daily dose is 3.2 g in 2 divided doses. The medication should be used in children over 8 years of age.

No dose adjustment is needed in elderly patients and patients with hepatic impairment.

In patients with impaired renal function, the dose should be determined based on the creatinine clearance rate:

Side effects

Central and peripheral nervous system disorders: hyperkinesia, exacerbation of epilepsy, insomnia, headache, ataxia, balance disorders, agitation, increased irritability, anxiety, confusion, hallucinations, increased libido, rare - drowsiness, depression, asthenia.

Gastrointestinal disorders: nausea, diarrhea, vomiting, abdominal pain.

Disorders of skin and its derivatives: dermatitis, pruritus, rash, urticaria, edema, including angioneurotic edema.

Others: individual hypersensitivity, anaphylaxis, hyperthermia, weight gain, arterial hypertension, injection site pain, thrombophlebitis.
